With most externs in .c addressed

I have caught most of them but there are a few more and some might have better
places after the next change to this driver.  There is work pending which will
be submitted after this is committed which switches the console driver to
libchip.  It adds a vgacons.[ch], replaces console.c, and encapsulates things a
little better.  So please let's review the externs more carefully after that
one is merged.
