There is only one RTEMS_offsetof usage that I see.  In the monitor code. 
Change that.  

Remove RTEMS_offsetof.  It was never documented and leaked.  We document this
in the 4.11 release notes and move on.

This is really minor.  This macro dates back to the dark ages.  Let's kill it,
document that killing and move along.
