New patch

Three bugs:
1) The last server entry was not removed
2) Entries were not cleaned up on a restart of DHCP (only on lease renewals)
3) rtems_dhcpd_nameserver_count was not reset in clean_dns_entries, and
therefore new DNS entries weren't added.

Here is a new patch.
