stm32h7-hal: Refactor clock and UART handling

- Introduce STM32H7_UART_TIMEOUT macro for polled UART operations.
- Refactor stm32h7_uart_polled_write to use defined timeout constant.
- Clean up stm32h7_gpio_init function and ensure type safety with RTEMS_DECONST.
- Organize peripheral clock enable/disable functions for clarity and bounds checking.
