[PATCH] Reference counting for file system locations

Hello,

sorry that this patch is so huge, but it is hard to get it smaller since the 
introduction of reference counting was quite a massive undertaking.

Please review and test this patch.  A fix for the file system in tfsDriver.c is 
not included (I will do this after the review).
