Add and use more fatal error sources

This change set adds more sources to Internal_errors_Source.  The application
exit via exit() is now also a fatal error.  A new application level function
rtems_fatal() is provided.

Each BSP provides now a default initial extension.  The application can
override it via <rtems/confdefs.h>.
