Each file should be still self-contained in terms of copyright and license.

I would like to have a file which lists everyone who agreed to the 
BSD-2-Clause license change.

The BSD-2-Clause license wants two things from a user of the software:

1. Redistributions of source code must retain the above copyright
    notice, this list of conditions and the following disclaimer.

2. Redistributions in binary form must reproduce the above copyright
    notice, this list of conditions and the following disclaimer in the
    documentation and/or other materials provided with the distribution.

Having this notice in one file makes it a bit easier for the users.

No, this should go into the RTEMS Software Engineering manual. I work on 
an update right now.
