[PATCH] rtems: Add "Notes" paragraph header

The problem is that this patch does two things:

1. It adds @par Notes.

2. It moves the notes and description to the same place as we have in 
the RTEMS Classic API Guide.

So in some blocks we just move a description if there are no notes. In a 
follow up patch set there will be additional changes to the notes since 
some of the notes move to the new constraints paragraph.
