What is going on with FTP?

We suspect the firewall is blocking the connect to the secondary port  after
using the primary port to make the connection.  We have so far been unable
to catch it in the act since it seems to work from within our network 
and from
the ISPs we have access to.

We don't think any configuration settings have changed.

If someone has suggestions on what to look at or test, please speak up.  
It will
certainly help get it resolved.
