__BSD_VISIBLE

Ok my understanding so far is that there are some features in BSD that are
incompatible with POSIX or X/OPEN. So __BSD_VISIBLE should be undefined
when using POSIX API.

I think you cannot globally enable it or not. It depends on what subsystem
you are using.
