File system changes

Hello,

I changed huge parts of the file system support code and the file system 
implementations.  The aim was to

	1. simplify the file system implementations (the path evaluation was
	   to complex), and
	2. to support file systems on removable media like USB sticks.

The changes affect all file systems within RTEMS.  The main commit is this:

All users who want to work with file systems in RTEMS 4.11 are encouraged to 
test these changes.

The file system API has changed considerable.  Users with own file system 
implementations must adopt their code for RTEMS 4.11.
