This issue captures review comments by @gedare in !283. The MR is the patches posted by Patrick to the `devel at rtems` list in 2017.

1. Change the mutex resources to be self-contained
2. Make debug printing conditional code and guard including `stdtio.h` on debug output being enabled
3. Use `printk` for debug output
4. Add SPDX tags to the start of the sources
