Doxygen Clean Up Patches

Hi

I have committed a handful of patches which improve the
Doxygen output.  The online version at rtems.org will regenerate
sometime overnight.

+ Groups more properly organized hierarchly so this page
     looks more like it should:

http://www.rtems.org/onlinedocs/doxygen/cpukit/html/modules.html

    This puts more under File Systems and POSIX. There is
    still some to fix and fine tune.

+ Many files had no EOL at EOF. Fixed that.

+ Many @defgroup had junk names.

+ Fixed many warnings in the input.

+ Found some formatting errors and fixed those.

+ Consistent use of syntax to start a group.

There are still LOTS of "undocumented" warnings. And
there are a few "special" warnings I will be pinging
individuals about over the next week.
